Did this start happening after an update? If so, make sure you download every update in order starting from the version you currently have installed. ex) version 1.11 installed would require you to download v1.12 and then v1.13.

If it just happened out of the blue, try reinstalling the game.

Are you running the latest snapshot or version 1.13? What type of controller are you using?
